YOUR PARENT OR GUARDIAN MUST COMPLETE THE CONSENT ONLINE AND THEY MUST EITHER BE PRESENT AT THE TIME OF VACCINATION OR PROVIDE THEIR SIGNATURE ON A PAPER CONSENT FORM. WE WILL NOT BE ABLE TO VACCINATE MINORS WITHOUT PARENTAL CONSENT. We appreciate your cooperation.
For patients receiving Moderna 6 months to 5 years old:
The Moderna COVID-19 vaccine for 6 month to 5 year old vaccine is a *2 dose primary series*. The required interval for Moderna COVID-19 vaccine for 6 month to 5 year olds is 28 days from the first to the second dose. Two (2) doses must be received for the primary series to be complete.
As of 12/8/2022, the Moderna Bivalent Booster vaccine has been approved. The required interval for Moderna COVID-19 Bivalent Booster vaccine for 6 month to 5 year olds is 2 months since the second dose of Moderna Monovalent Vaccine for 6 month to 5 year olds.
For patients receiving Moderna 6 years or older:
The required interval for Moderna COVID-19 vaccine for 6 years to 11 years old is 28 days from the first to the second dose. 2 months after second dose you may get your bivalent booster dose. If you qualified for a third dose 28 days after second dose because you are moderate to severely immunocompromised, you may schedule a bivalent booster dose 2 months after your third dose.
The required interval for Moderna COVID-19 Bivalent Booster vaccine dose is 2 months from the completion of the primary series or the most recent booster dose of any other COVID vaccine.
You must have already completed the primary series of COVID vaccine before you may receive a dose of either COVID Bivalent Booster vaccine.
For patients receiving Pfizer 6 months to 4 years old:
The Pfizer COVID-19 vaccine for 6 month to 4 year old vaccine is a *3 dose series* with the third dose being administered as the Pfizer Bivalent COVID vaccine. The required interval for Pfizer COVID vaccine for 6 month to 4 year olds is 21 days from the first to the second dose, then 56 days from the second dose to the third bivalent dose. All three (3) doses must be received for the primary series to be complete.
Please be sure you are scheduling the correct Pfizer vaccine based on the child's age at the time of vaccine administration. If a child turns 5 before their next dose of Pfizer vaccine, they may have either dose (6 month to 4 year old vaccine OR 5-11 year old vaccine) of Pfizer for their vaccination.
As of 12/8/2022, the Pfizer Bivalent Booster vaccine has been approved. The required interval for Pfizer COVID-19 Bivalent Booster vaccine for 6 month to 4 year olds is at least 8 weeks since the second dose of Pfizer Monovalent Vaccine for 6 month to 4 year olds. Those who have already received a third dose of the Pfizer Monovalent COVID vaccine for 6 month to 4 year olds may not receive an additional dose of the Pfizer Bivalent COVID vaccine at this time.
For patients receiving Pfizer 5 years or older:
The required interval for Pfizer COVID-19 vaccine for 5-11 years old and >12 years old is 21 days from the first to the second dose. 2 months after second dose you may get your bivalent booster dose. If you qualified for a third dose 28 days after second dose because you are moderate to severely immunocompromised, you may schedule a bivalent booster dose 2 months after your third dose.
Please be sure you are scheduling the correct Pfizer vaccine based on the child's age at the time of vaccine administration. If a child turns 12 between the first and second dose of Pfizer vaccine, they may have either dose (5-11 year old vaccine OR 12+ vaccine) of Pfizer for their second dose of Pfizer COVID-19 vaccination.
The required interval for Pfizer COVID-19 Bivalent Booster vaccine dose is 2 months from the completion of the primary series or the most recent booster dose of any other COVID vaccine.
You must have already completed the primary series of COVID vaccine before you may receive a dose of either COVID Bivalent Booster vaccine.
For patients receiving Janssen vaccine:
You must be 18 years or older to receive Janssen COVID-19 vaccine. You may NOT receive more than 2 doses of Janssen COVID-19 vaccine, then you must switch to Moderna or Pfizer Bivalent COVID vaccine. The required interval for Janssen COVID-19 vaccine doses is 56 days from the first to the booster dose. It is recommended that you choose Moderna or Pfizer Bivalent COVID-19 vaccine for your booster dose if you previously received Janssen COVID-19 vaccine for your primary dose.
For patients receiving Novavax vaccine:
You must be 12 years or older to receive Novavax COVID-19 vaccine for your primary series of COVID-19 vaccines. For primary series, Novavax COVID-19 vaccine is only approved to be given to those who are previously unvaccinated against COVID-19.
The required interval for Novavax COVID vaccine primary series doses is 3 weeks (21 days) from the first to the second dose.
The Novavax COVID-19 Vaccine BOOSTER dose is approved ONLY for those who meet ALL of the criteria below:
• At least 18 years of age
AND
• Completed a primary COVID-19 vaccine series with any FDA-approved or FDA-authorized monovalent COVID-19 vaccine
AND
• Has not received any booster doses of COVID-19 vaccine
AND
• Unable (e.g., mRNA vaccine contraindicated or not available) or unwilling to receive an mRNA vaccine; would otherwise not receive a COVID-19 booster dose.
If you meet ALL FOUR of the above criteria, you may receive Novavax as your booster.
Note that if you received two doses of Janssen, first dose was considered primary series completion and second dose was considered booster, OR if you have received one dose of Janssen followed by a dose of either Moderna or Pfizer vaccine, you have already received a booster dose and are not eligible to receive Novavax as an additional booster.
Please arrive a minimum of 15 minutes prior to your appointment time.
REMAIN 6-FEET APART.
The vaccine process will take approximately 10 minutes. You will be asked to wait onsite or in your vehicle if it is nearby for at least 15 minutes after your vaccination before you leave to ensure you have no adverse reactions.
